HEALTH & COVERAGE
Sport Accident Insurance
Supplementary coverage for eligible expenses not covered by MSP or your extended health plan. You need to use your MSP and extended health first, and then our insurance may cover some of the remaining expenses.
Claims must be submitted within 90 days of the accident. Late claims are reviewed at the insurer’s discretion.
How to submit a claim
- Download the Athletic Accident Claim Form.
- Complete the form and include the supporting documents listed on page 2. Send everything to ed@vul.ca or Box 20093, Fairview PO, Vancouver, BC V5Z 0C1.
- Vancouver Ultimate will confirm your eligibility and inform you of the next steps.
